Home Health will perform the following:

• Identify Waiver eligible patients

• Validate home environment has electricity and phone service

• Co-develop and implement a service plan including the type, mode and frequency of service

• Coordinate participants service plan with the AAA service plan

• Provide an Informed Consent Form to Participant

• Provide LPN or RN responsible for evaluating participant data and monitoring the service

LifeSignals will perform the following:

• Supply the monitoring equipment

• (Android Tablet, Blood Pressure/Sugar Monitor, Weight Scale)

• Supply the Monthly Cellular Data Plan

• Monitor the data

• Co-develop the service plan with At Home Health staff

• Send alerts and alarms to XYZ Home Health

• Provide messaging services to route calls and alerts/alarms
